NEW CISD Enrollment

Account Request

This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
IMPORTANT: This enrollment application is for NEW to CISD students and former CISD students who are enrolling for the first time this year ***Applications for the 26-27 school year that are submitted prior to April 1st will be canceled. You will need to complete a new one once the 26-27 enrollment window is open on April 1st** While completing the application, you will be required to submit the following documentation. You may scan them in or take a photo of them with your phone to upload, either will suffice. 1. Copy of the child's birth certificate. 2. Copy of the Child’s Social Security Card 3. Withdrawal paperwork from the previous school attended, if available 4. 2 Proofs of Residence (A current water, electric or gas bill, a lease, deed/title, property tax statement, or builders contract; no disconnect notices or telephone bills will be accepted). 5. Driver's License or State ID of person enrolling student. (If address matches utility bill or lease, Driver's License or State ID can be 2^nd proof of residency) 6. Current Immunization Records.
