This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Complete required fields to request an account to enroll your students. If you don't have an email address, plus obtain one from free service (Google, Yahoo, Outlook, etc). If your child is already attending school in Cudahy, this is not the process to use for verifying enrollment. This is for BRAND NEW to Cudahy students. Please go to our website under Families and select Skyward Family Access.
