New Student Enrollment: Account Request

This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure Online Enrollment system.
While completing the student registration, you will be asked to upload attachments of the Birth Certificate, Proof of Immunization, Proof of Residence, Photo ID, child's last Dental Exam, and well-check paperwork (Physical Exam). For your convenience, it may be helpful if you have the required documents on your computer to add as attachments. Complete required fields to request an account to enroll your students. If the student you are registering is transferring from another school please make sure to fill out the authorization to release form. PLEASE NOTE FOR NEW KINDERGARTEN REGISTRATION: While completing the student registration, you will be asked to upload attachment of the Birth Certificate, Proof of Immunizations, Proof of Residence, Photo ID, Dental and Physical Forms. These documents are required for the start of the school year but may be submitted to the school as you have them available between now and the first day of school.
