This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Please utilize the form below to enroll your child(ren) in Johnsburg School District 12. If you already have a child enrolled in the school district, or if you have previously had a child enrolled in the school district, please DO NOT use this form. Alternatively, please log into Skyward Family Access using your assigned Login ID and Password and select "New Student Application" from the sidebar. If you do not remember your Login ID or Password, please click the "Forgot your Login/Password" link on the login page.
