A. a student’s name B. photograph C. participation in officially recognized activities and sports D. height and/or weight, if a member of an athletic team E. date of graduation F. degrees and awards received Parents and eligible students may refuse to allow the Board to disclose any or all of such "directory information" upon written notification to the Board within fourteen (14) days after receipt of the District Administrator's annual public notice or enrollment of the student into the District if such enrollment occurs after the annual public notice. Any parent or eligible student who refuses to allow disclosure of directory data and who participates in the extracurricular activity must complete the appropriate acknowledgement, which includes a limitation on the refusal to disclose directory information obtained during the course of the student’s participation in extra-curricular activities. If you wish for your child(ren) to NOT have their picture submitted to our local paper, on our FB page or website, please send an email to gatzkeb@johnsoncreekschools.org. Be sure to include the first and last name of each child that you wish to exclude. For further information regarding Directory Information and FERPA, please go to our annual notices on the Johnson Creek School District website for your reference. Insurance Waiver: JCSD does not provide any type of health or accident insurance for injuries incurred by your child at school or while participating in a school activity/sport. |
