This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure Online Enrollment system.
Please complete required fields below to request an account to apply for enrollment at Meadowbrook Academy for the 2025-2026 school year. You must enter a valid email address. Open Enrollment begins on Monday, February 10^th. No applications will be accepted prior to this date. Please note, due to the large number of applicants on our waiting list, we are no longer accepting K5 applications for the 2025-2026 school year.
