This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure Online Enrollment system.

BY CONTINUING THE APPLICATION PROCESS YOU ACKNOWLEDGE** As the parent or guardian of a transfer student, I understand that the school corporation may discontinue the student’s enrollment either immediately or in a subsequent school year if the student has been suspended for more than ten days, as a result of possession of a firearm, deadly weapon, or destructive device on school property, for causing physical injury to a student, school employee, or visitor to the school, for violating the school corporation's rules regarding drugs and alcohol, or for a history of unexcused absences.
