This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure system.
Enrollment is completed one time, prior to a student ever attending OTHS. FAMILIES WITH AN EXISTING SKYWARD ACCOUNT LINKED TO ANOTHER CHILD SHOULD USE THAT ACCOUNT TO ENROLL YOUNGER CHILDREN. Complete the following required fields to request an account to enroll your students only if you do not have an existing Skyward account linked to another child.
