New Student Enrollment: Account Request
This form is the first step to enrolling your new student online. Complete it to request an account that you will use to log in to a secure Online Enrollment system.

  • Families interested in taking advantage of our Limited Open Enrollment will need to complete an application process and meet certain criteria.

  • This form MUST be completed and APPROVED, BEFORE you complete the new student online enrollment process.
 
Complete required fields to request an account to enroll your students.
 
**Required documents to be uploaded for enrollment:
  • STUDENT’S BIRTH CERTIFICATE
  • STUDENT’S SOCIAL SECURITY CARD
  • STUDENT’S IMMUNIZATION RECORDS
  • GUARDIAN STATE ID
  • CURRENT PROOF OF RESIDENCY SHOWING PARENT/GUARDIAN'S NAME & SERVICE ADDRESS
Acceptable Proof of Residency Documents
For the purpose of establishing residency, parents or guardians must provide one of the following documents, which must be dated within 60 days of completing registration:
  • Mortgage statement showing the parent/guardian’s name and address
  • Signed lease agreement showing the parent/guardian’s name and address
  • Utility bill (electricity, water, gas, or cable) showing the parent/guardian’s name and the service address (not mailing address)
Unacceptable Proof of Residency Documents
For the purpose of establishing residency, the following documents are not accepted (including, but not limited to):
  • Phone bills or statements
  • Trash service bills or statements
  • Property tax statements
  • Auto or home insurance statements
